Insurance:
Insurance Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

  • What are my health insurance coverage and benefits?

  • What is my deductible and has it been met?

  • Do I have a co-pay? If so, how much is it?

  • What is my wellness visit coverage?

 

 

Cancellation Policy:

If you do not show up for your scheduled appointment and you have not notified us at least 24 hours in advance, you may be subject to cancellation fees. Schedule online, request an appointment online here.

Additional Fees: 
 
Co-Pays assigned by private or commercial insurance carriers must be paid at the time of service for office visits, the amount of your copay should be indicated on your medical card. Medicare usually requires a 20% share of services be paid by the patient. We will bill you for that amount once Medicare has processed the Claim.

Doctor Trip Fees for Home or Domicile visits.
 
Trip Fees for House Call Visits are not covered by insurance. We therefore must charge a nominal fee for the trip to see a patient in their place of residence. (Average cost, $35)

 

There are additional fees for completion of forms for use outside of the patient's medical record, including:

  • Medicaid Waiver Programs

  • Pre-Employment

  • Employment/Workman's Comp

  • Life Insurance.

  • Family Medical Leave

  • Hard copies of Medical Records

  • Family Consultations

  • Advanced Directives Formation

Uninsured Patients and those who will be required to pay through private means will be accepted on a case-by-case basis.

We do not offer boutique or concierge medicine.
